David Adam<p>With five week's warning, the Australian Medicare arrangements for chronic disease management are being basically upended.</p><p>It's important to remember that the primary function of the CDM program is to encourage primary care general practitioners and general practices to direct bill (bulk-bill) Medicare rather than charging patients who could then claim a rebate. It was introduced as direct-billing rates started to slide in the 90s and successfully arrested it.
